Extended Care Protection Plan (“Plan”) is an optional Plan offered to one or more members of the household(s) who wish to be covered for head treatment services. Everyone in one (or more) household(s) gets screened for $15 each and anyone that has lice, gets treated at a discounted rate and then all qualify to enroll for $9.95 per person monthly fee. Anyone or everyone can sign up at that time. Only those who are active members of the Plan are eligible for Plan benefits. Others pay full price.

ENROLLMENT FEE

  • $10 per household. (Waived if a household member is paying for a Signature Lice Removal Treatment at time of enrollment.)
  • $9.95 per person, per month, for a minimum of 12 consecutive months.
  • If any family member is positive for lice and declines Signature Lice Removal Treatment at the time of enrollment, that automatically disqualifies all family members from enrollment.

OVERVIEW OF THE PLAN

  • All members of the household and secondary households (where applicable) must be screened and cleared of lice and/or treated by Lice Clinics 805 within 24 hours of original treatment date to be eligible for the Plan.
  • Whenever a Plan member is receiving a Plan treatment, every member of the household AND secondary households (where applicable) must again be screened and cleared of lice and/or treated by us within 24 hours to continue the Plan. Non-members pay full price or discounted rates they qualify for. Plan members get $5 screenings.
  • Member must be currently enrolled in the Plan and current on all payments to receive Plan benefits. NO exceptions.
  • Once the Plan member(s) receives three (3) Plan treatments within 12 months, the member can pay a re-enrollment fee of $125 and sign a new 12-month plan for $9.95 per person, per month.
  • Plan cannot be “frozen” and reinstated later without incurring a $125 fee for re-enrollment plus monthly membership fees of $9.95 per person for each month that was missed. Each member of the household(s) and/or family must be screened and/or treated at regular price to qualify for re-enrollment.
  • Credit card information will be safely stored and charged automatically once a month for the Plan fees. See enrollment forms for all details.
  • We reserve the right to refuse enrollment or terminate enrollment of our Plan for any reason and will not refund any portion of paid fees. The Plan is subject to change. Members will receive emails with 30 days advance notice of any Plan changes.
  • Member will alert Lice Clinics 805 of any additional new people that move into household within 24 hours of moving in and will bring the new household member for a paid screening within 24 hours.
  • After the initial 12-month period, this agreement is on a month-to-month basis and may be cancelled with 30 days written notice from either party. No refunds or credits will be issued under any circumstances.
  • Limitation of Liability. In no event will Lice Clinics 805 liability for damages to Member exceed the amount paid by Member to Lice Clinics 805 or parent company.
  • Agreement goes into effect 48 hours after Agreement is signed and first payment is processed.

    CAREFULLY READ this Subscription/Membership Services Agreement (“Agreement”) and Extended Care Protection Plan (“Plan”) before you sign it. Lice Clinics 805® (“Owner”) is only willing to provide the services to you (the “Customer”) if you agree to the terms and conditions in the Plan and Agreement. Signing this Agreement forms a binding legal contract with Owner.
  • Services & Benefits. Owner will provide services & benefits as defined in the Plan to each person enrolled in the Plan, while the account is active and payments current. Guarantee. The Signature Lice Removal Treatments are guaranteed to leave the treated person lice free with a 15-day guarantee as long as each person living in the same household(s) and/or secondary households (where applicable) have been checked, treated and/or cleared of lice within a 24-hour period. Otherwise, the Signature Lice Removal Treatments are not guaranteed, and the membership may be cancelled. Services for Persons Not Covered by This Agreement. All standard and applicable charges apply for services provided to those who are not listed on this agreement, including screenings. Customer will alert Owner of any additional new members that move into household within 24 hours of moving in and will bring the new household member for a screening the same or following day. Fees. Customer agrees to pay a non-refundable amount of $9.95 each month for each person covered by this Agreement. Term. This agreement shall continue for a period of 12 months and continue month-to-month thereafter until terminated by either party via written 30-day notice unless all Plan treatments have been used, in which case Customer may pay $125 re-enrollment fee for another 12 months. Cancellation. Early termination fee of $125 will be charged to Customer’s credit card on file. After the initial 12-month period, this agreement is on a month-to-month basis and may be cancelled with 30 days written notice from either party to either party. No refunds or credits will be issued. Limitation of Liability. In no event will Owner’s liability for damages to Customer exceed the amount paid by Customer to Owner in the immediately preceding 12-month period for this Agreement. Costs Subject to Change. Customer understands that the Agreement cost may be subject to change. Customer will be notified at least 30 days in advance of any changes in the amounts to be charged and will be provided the opportunity to cancel this authorization at that time even if it’s within the first 12-month Term without any early termination fee. Credit/Debit Card Authorization. Customer hereby authorizes Lice Clinics 805 (aka Tresis Partners Corporation) to process a one-time $10 enrollment fee followed by monthly charges of $9.95 per enrolled member, who is covered by this Agreement, for a period of no less than 12 months. Payment. Customer understands the Agreement/Plan fee will be processed each month that this Agreement is in effect. Payment will be made automatically on a monthly basis through our Point of Sale system. If payment is declined, Customer will be notified via email or phone on file. Non-payment may result in removal from the Agreement/Plan or clinic declining benefits. It’s the Customer’s responsibility to update Clinic of changing information. Effective Date: Agreement goes into effect 48 hours after Agreement is signed and first payments are processed. Initial & Follow Up Screenings. Each member covered by this Agreement/Plan must be screened and cleared and/or treated by Lice Clinics 805 at the time of enrollment and/or after any re-treatment of any member under the Plan (at full price) unless they are a member of the Plan.
